Students are not permitted to carry or self administer medication in school, this includes prescription and over the counter medications such as Tylenol or Motrin. If your child may need to receive medication in school, please have his/her physician complete the medication form, a parent signs the form and sends in a small bottle of the medication ordered. The medication sent in must be the same dose as the physician ordered, or I will not be able to administer medication. You must send in the medication since I do not keep stock medication for students. If your child feels they need medication, they can come to see me. I will assess them, and medicate if indicated.
The only exception to this rule is if your child is permitted to carry an epipen-for severe allergic reaction, or an inhaler-for asthma. If your child will be carrying either of these, print and have their physician complete the appropriate allergy or asthma medication form. If your child is permitted to carry and self administer, please make sure the physician indicates that on the form. The completed form is to be submitted to my office. If you would like me to keep an epipen or inhaler in my office, please supply that.
 
If your child has an annual physical, please send it in to me along with a copy of updated immunizations. This will keep me informed of any health concerns. It is important to keep your child's immunization records up to date especially for tetanus boosters and the new immunizations available for adolescents.
